Steps for Regular Season Registration
Checklist for Registration
- Register with USA Hockey
- Register with Mid-States Club Hockey Association
- Complete Mid-States Club Hockey Association's Medical Consent Form and turn into Duchesne Hockey Club Head Coach
- Register for Duchesne Hockey club Varsity and/or B Team season.
- Submit players last semester report card and send to dhcregistrar@gmail.com
- If you are a 2009 birth year or earlier, you will be required to take SafeSport Core Training or Refresher Course prior to participating for the 2026-27 season, SafeSport Certification
Duchesne Hockey Club Requirements
All club players must read, sign and return a copy of the Duchesne Hockey Club Players Code of Conduct. Form provided by Head Coach.
Sibling Discount - Contact DHC Treasurer before registering.
A deposit payment is due at time of registration. Payment plans are available at registration.

Mid States Requirements
Academic Eligibility - All Varsity/B-Team players need to have their previous school year's report card or transcript uploaded prior to the start of practice. This can be done as part of the club registration process. Mid-States requires that all high school players have passed at least three credits of classes in the most recently completed semester.
